The upper section of the form below contains your Membership Information and will not be shared with anyone outside of the PSFA.
The lower section will be published in the Member Directory which will be distributed to each PSFA member. It’s a great way to stay connected to all of your child’s friends who might be from all corners of Monmouth County.
Enter information EXACTLY as you’d like it to appear in the Directory. Proper capitalization, nicknames (ie, if your son Robert prefers “Bob”)
Please make sure to fill all REQUIRED fields (marked with an asterisks *). The form won’t process with any empty required field. If you have a popup blocker on your computer, you may not see the error message. Some cell phones have trouble with this, too. If you don’t get a confirmation email immediately after your register, it didn’t go through.
In order to be eligible for one of the ten $500 scholarships given away senior year by the PSFA, you must be a member for all four years.
If your child ever decides to leave CHS for any reason, we will prorate and refund the balance of your dues. So feel free to enroll for all four years with confidence.
